"The training was extremely helpful, we were taught the old school way buy using scenarios that stick in your mind. We were using the ID Now as a confirmation on negatives that were run on a rapid kit. We found about 35% of negatives on rapid tests were positive on the ID NOW!"

"This is a great instrument. Wonderful during the flu season. Would love to see a different color than gray for the invalid response it would alert the tech from across the room that there was an issue that they need to address right away. The gray makes them think it is a negative result. This is my only issue with the instrument. Would like to see more testing available on it soon."

"Our installation process was rushed due to our clinic being short staffed and the trainer had another appointment. Our nurses are still having frequent invalid test error. We have requested additional training but have not received this. The cost per test is high when it frequently takes 2 tests and sometimes we still have to run a test with our regular kit testing."

"Overall, the product is user friendly. Have had to replace the machines several times due to malfunctions. Twice when we received a replacement, I had to get a replacement for the brand new ones due to malfunction. Our office has had several invalid results which in turn costs the office money. Kits are expensive and reimbursement is not always enough to cover cost of test kits."

A regular error message is "Unable to Analyze" without further explanation

"The testing cartridges were very confusing for my staff, especially when we are using similar testing materials for the Strep test. I can't quantify how much of the testing supplies were wasted because the staff grabbed the wrong supply. I don't think much was done to differentiate the testing materials. In addtion, the reimbursement was low. I actually lost money doing these tests in my office."

"The machine is easy to use. It explains every step very well. My issue with this machine is reliability. The software has a tendency to freeze and the only way to finish the test was to start over. We went through many tests that were not needed because of that. The reagent cartridges are expensive and we had a lot of waste that way. I like the machine as far as ease of use."

Great little instrument for molecular testing

"Instrument is really easy to use, reliable results, fast turnaround times for ER testing. The only issue is the instrument itself is less than robust as far as reliability is concerned. We have a backup instrument, and I would recommend anyone using this system to have a backup. This is a great little molecular testing instrument, and I am anxiously awaiting further assay development."

Not as easy as it looks.

"The fact that it is CLIA-waived is a big draw. The "fool-proof" color and shape-specific pieces for each step make it look like it can't be done wrong. Wrong. Most of the "trained" users have managed to fail to get the pieces put together correctly, and we have wasted most of the reagent while failing to get test results. That being said, when a result is produced, we have every reason to trust it is accurate. We got rid of it in favor of a Cobas LIAT instead."

Fairly easy to use and I like the way it takes you step by step on how to use Alere

"Our office is fairly new to Alere. We have had some apprehensions due to the wait time for each test and how it worked. We had a rep who came to our office and demonstrated how to properly use this machine. Fairly easy to use and I like the way it takes you step by step on how to use just in case you forget :) Also like the collection process -less is more! We will be using Alere for other tests done in office. At the moment influenza is the test being ran. Time frame for testing is the same we were using with another product. Difference is how we swab (less is more) and should be more accurate. As for reimbursement rates -that is yet to be seen."

Great for laboratorians, a bit "complicated" for non-laboratorians

"We use this analyzer in multiple locations. In our main lab, with medical technologists as the operators, we have very little to zero issues with it. However, in our physician offices, with nurses as the operators, there are a lot more problems. Clearly it's an operator issue, but I would say "ease of use" could be better since it's problematic for non-laboratorians. Making sure the orange button fully engages and disengages seems to be one of the main issues, even after multiple trainings and re-trainings. The other problem is they go months at a time not using it, when we're out of flu season, so every year it requires training all over again because they forget."

"The machine is easy to use with little to no up keep. Results are pretty quick and easy to interpret. Does not require a lot of training and the step by step video instructions make it idea for seasonal testing. You don't really have to brush up to much on the procedure so you are ready to go from the first flu to the last flu. The only aspect of the machine I have had trouble with is the touch screen. They tend to die out after a while and there really isn't a way to fix them besides calling and getting a new one. Not that this is difficult as they make the process super simple and fairy quick."

"We have used the Alere i in our office for over a year and we absolutely love it! It is user friendly and gives accurate results. Parents really value knowing if their child has actual influenza, and not having to go to the hospital lab to find out. It is also nice that they leave our office with results, no call backs the next day. Collecting a sample is easy, even on the smallest patients. I cannot imagine going through an influenza season without this test. Our office loves the technology so much that we are starting strep and RSV testing next week. Our staff is beyond thrilled to stop running throat cultures any more. This will be a huge time saver for us and allow us to better treat patients with strep throat. Every office should have an Alere i!"

Good instrument, but finicky.

  • Application Area:We've been using Alere i for influenza testing for the past 2 years.

"Reimbursement has been very good so even though the tests are more expensive, we have seen higher profits with this flu test compared with other quick tests. The customer service is quick, even willing to send reps into our office to help set up and trouble shoot the equipment. The downside is that the machine is very sensitive. We have experienced significant difficulty with invalid tests - over 15%. Reports have been run from our machines and the company still wasn't able to help us figure out the reason why."

"We use the alere I for strep, flu and rsv. The machine is easy to use and gives reliable results. What I like most about the technology is for a rapid waived test the results are not questionable. What I like the least about it is the amount of space the kits take up."

Love moving from an antigen rapid test/manual read test to a molecular testing system

"We wish that the strep A test did not require a strep culture to verify negative results from the analyzer. There are a few test systems that do not require this culture followup which is great for the user end of things. We know that we can perform a study to prove or disprove moving away from this culture but just a thought that would please the customer and have advantages over other companies with similar test systems. Also, we are not a fan of the walk away then come back 3 minutes later type of set up. With todays technology many things are indeed "walk away". It is not that hard to set a timer and come back to do the rest of the testing but when you work in a busy lab that is only staffed with 1 tech 70% of the time. It makes it hard to be everywhere at once. If we could just load and go. That would be optimal. Just a thought."

We get too many "invalid" results resulting in wasted time/money!!

"Us nurses are not happy with this machine most days. We get too many invalid results which wastes our money and time! Doctors want results asap, not after trying 2-3 times. We have tried so many times on some patients that we have to re-swab them! That is inconvenient all around!!"

"The machine is fairly easy to use, there are a few staff members who don't push the cartridge all the way down and then it doesn't tell you that you have done it incorrectly until the 10 minutes is up. It would be nice if it just wouldn't run the test and tell you that right then. I do love that the swab is only nasal, however, rather than nasopharyngeal. The patients appreciate that as well."

"It is easy to use and the results are reproducible. Get quick service or help if issues arise. When we needed one replaced, it was sent out that day so we were not down for any length of time. The woman who came to set them up at all of our sites was professional and very helpful. Did not have any major surprises after we were on our own (running without trainer). Well made so we have only had to replace one or two over the years."

"We use the instruments but become frustrated sometimes with the "invalid" results and also that we have to replace them so often because the touch screen doesn't work. We have a total of about 15 machines and I think most of them have been sent in for replacement. They sometimes make grinding noises and we have had to replace a couple machines because the motor no longer works properly."

Too many invalids due to "pipetting" issues caused us to stop using instrument

"As a POL manager, we had several nursing and laboratory staff using the instrument, the number of steps to perform the test along with high incidence of invalids caused us to stop using this instrument. Even after contacting technical services and sending in information from the instrument, the answer we received that we were having "pipetting" issues. With so many different users resulting in the same error, to me that is due to the testing procedure or kit issue."

"Alere i is a CLIA waived test that easy to use. The integrated, rapid, and simple characteristics of the Alere i Influenza A&B assay make it a very good candidate for point of care testing. It's turnaround time is quite good, but for us when both an influenza A&B and a Strep is ordered it does take a little bit longer than other rapid tests. The Alere i utilizes NEAR technique to detect and distinguish influenza virus A and influenza virus B. It is a novel isothermal-amplification-based integrated system for detection and differentiation of influenza virus. It is rapid and quite user-friendly procedure with a time from specimen collection to results of less than 15 min, including the 21 min hands-on time, but the time is extended when for us as our controls are done date of testing not daily. The Alere i Influenza A&B serves us as an alternative to our rapid influenza antigen detection tests with significantly improved sensitivity. It has the advantage of a significantly shorter test time than any other currently available molecular assay. The simple pipette free procedure runs on a fully integrated, closed system. It also has a small footprint and is CLIA waived."

"Instrument is so easy to use and easy to train new users on. Troubleshooting with the company has been a positive experience, though it did take awhile for our data to be analyzed. The only issue we ever had was determined to be from improper collection (collecting samples with too much mucous interfered with testing). The step-by-step instructions on the screen are great and really help the user test, especially those who may not use the instrument very often. The training documents are great and the pre-prepared SOPs and IQCPs made it easy to implement in our lab. We have had it for over a year and love it."

"I've had this device in all 36 sites so far and this is MY experience: Keep doing this: User friendly, "training made simple" device, prompts are clear and self explanatory. Consumables are durable and sealed before use, ensuring both efficiency and accuracy. Customer service and the easy replacement policy is A+!By far one of my most favorite analyzers. What needs to work: The screen freezes inexplicably, units have been replaced often (at no cost, but it is disruptive to workflow). Is it possible to add an alarm for consumables accidentally left behind? This would be helpful for the A1c, as well."

"We struggled with this analyzer and only used it for a few months before we switched it out with a competitor that was much easier and more reliable. We had trouble with the machine reporting out results and often had the machine fail half way thru the test. We would have to turn the machine off and re-start completely. We had to collect multiple patients samples just in case of failure. Wasn't worth the hassle and the expense of using multiple kits to get a result."

Very sensitive&specific method; limited to only one test at a time, expensive method

  • Application Area:Detect nucleic acid of Influenza A and Influenza B in symptomatic patients

"The nucleic acid Influenza method is very sensitive and very specific. The instrument is quite simple to use. There are a few specimen processing steps that take place at specific times that require tech attention, but it is not too bad. The instrument also just a single-test analyzer, so it is not likely a best fit for a busy laboratory unless multiple units are purchased. We had an issue with the touchscreens on two previous instruments that rendered the instrument non-functional, but Alere replaced each unit overnight. As with any molecular method, it is not an inexpensive test, but it is nice to obtain reliable results. Proper specimen collection is very important--specimens collected with heavy amounts of mucoid nasal secretions will cause the results to be invalid or indeterminant, requiring recollection or testing with an alternate (EIA) method. I would advise collecting from the nostril exhibiting less drainage."

"There are multiple steps in running the samples. If one step is done incorrectly a whole test is wasted and they aren't cheap. Our main problem was closing the lid seconds before it advises you too, which in turn gave us invalid results. A definite error in the making."

"We get very good results, we back every negative test up with culture and have yet to have a false negative result. It's very quick and it is pretty amazing that you can receive a positive result in as little as two minutes. QC is easy and is included which is a bonus. The step by step walk through on the LCD screen makes it very easy to use and most new techs only have to see it once to be able to perform test on their own. I do wish it looked for Group C Strep as well."

"Great product. Much more accurate and reliable than visual results from rapid influenza tests. Very user friendly especially since the machine practically walks you through the entire process with pictures! Exceptional results with little to no error with the machine. Most errors (95%) come from the user. Just follow the pictures and instructions on screen and you will never fail. Great bang for the buck!"

"The Alere I is a great instrument for testing our pediatric patients for influenza. It is easy to use and we receive very accurate results. It is important for others to know that there may be a short transition period because it does take a little longer to run the tests, but it doesn't take long to establish a work flow that incorporates use of the Alere and it is well worth the change in work flow for the increased accuracy. We have had excellent after-sales care if we have had any questions. We are currently looking into how to integrate the results into our EHR directly as well."

"This product is fairly easy to use. The one down fall is that there are times when we do get invalid results multiple times. We are doing all of the steps correctly but still get invalids. I wish that the machine could inform us on what we are doing wrong and what can be done differently to not receive the invalid status."

"We mad the switch from a kit test to the molecular test, mostly because of the new FDA requirements. However, since having the instrument in house, it is very reassuring to know that we are giving the providers a high quality result for the benefit of the patient. Sometimes the cost of better technology is too high

"This methodology is expensive to test and in today's times the reimbursement is difficult to assess. The purpose of changing and upgrading to this molecular method was to keep up with the technological advancements, but it appears to place healthcare reimbursement further out of reach. The old system required a throat culture follow-up and this one states in the insert to do the same so if you follow the insert all you may have upgraded your technology, but you have essentially just increased the cost to the lab and the consumer."

"Results take a full 12 minutes, easy to use except for when transferring sample to test cartridge.... if not clicked properly error will show up after 12 minutes, many occurrences of this between all our clinics. Expense vs reimbursement was fair but patients that had cost rolled over to them b\c of deductible where not thrilled."

"Accurate and rapid results. The Physician can have a result and make a treatment decision before the patient leaves office. The machine is easy to use once staff is trained. The machine gives on screen images and walks the staff through the steps. Reimbursement for commercial insurances is good. Medicare usually covers cost of the test and Medicaid can be a bit low. The physicians care more about getting the correct treatment than losing a few dollars on Medicaid patients."

The only concern is that we have had to replace 7 instruments in the two years of our contract. "It would be nice if there was a ding at the end of each step so an additional timer wasn't needed. analyzer is too quiet and if you get busy you can miss the next step. Also it would be great if all the flu parts could be room temperature. The screen tends to freeze up from time to time. We have had to send a few back for replacement, not sure how long they are supposed to last. "

"The machine is easy to use, it tells you step by step what to do and when to do it...... now some of our employees have tried to speed up the process at first and the machine would say results failed, but after talking with TECH support and sending in data, it all came back to rushing the machine..... so you have to take your time and do as machine says to get the right result in the end..... very user friendly I feel that the results are right on, and you don't have to worry about a false positive with this machine"

"Results are quick and high quality. Use is quite easy, we had new staff come on that were trained but there was still a glitch. We contacted support. They were on the phone immediately, we forwarded documentation to them, their team conducted an investigation. Results were received within 2 hours of start of call initiation. Turned out to be user error with a simple fix. We have had no problems with reimbursement for it's use either. The company was great to work with."

"We removed Streptococcus reflect cultures by using this platform. We had 4.2% positive cultures that were negative on our previous strep antigen test, after implementing the Alere we have had no false positives. I really like how it prompts the user through the steps as well as it's fast turnaround time for a molecular result and its success rate of 0% false positives at our facility."

As a infrequent operator, I must check myself against the instructions each time I run a test, but the instructions are clear and the analyzer itself prompts each step. I do not recommend this product. Too many invalid test results.

"This product is great when it's not giving invalid results. The guidelines for not receiving invalid results are too extreme, especially for throat swabbing. Don't touch uvula, cheeks or tongue when swabbing throat. Also toothpaste, cough drops and other variables can cause invalid results. If you've ever swabbed a child's throat you will know how difficult this task is. If you receive and invalid test result, then you have to swab the patient again and run a different way and you can't charge a patient for an invalid test result. This costs the clinic money and makes the patient very angry. For flu swabbing there can not be any trace of blood on the swab, visible or microscopic. Our clinic has used 4 kits total of each test flu and strep and received a total of 18 invalid test results and there are still 10 tests left in the last flu kit to be used. 12 invalid flus and 4 invalid streps. We will finish using these kits only because they are so expensive."

"The product was presented in a professional manner with a test period for determination of purchase of the product. Training and testing went well. We given the insurance codes with the assurance the companies were paying well. After using the product during the flu season first we noticed a 6% fail rate (test results given as failed or when opening the test packages they were damaged. Then we started receiving rejections from the insurance companies. When we informed the company we were told at that time that the 6% was normal and had an explanation for the rejects from the insurance. We were told at that time that we needed to look at every insurance before using this test to be sure that insurance company would pay. Over all we lost about $3000 . The company did offer more product to compensate but this would not have solved the problem because we would still have losses as we continued to use the product. "We were so excited to get our Alere I setup for in office influenza testing. The device walks you through the instructions, so although we had a trainer in to instruct us, it was pretty easy. It takes about 15 minutes. Unfortunately, when it was billed to Ohio Medicaid, they denied payment. The test is CLIA waived but apparently Ohio Medicaid has not updated their database and payment was denied saying that we do not have the proper CLIA certification. We were advised to wait and re-submit in a few weeks. It remains to be seen whether or not we are paid for the testing. "

"We are live using the Alere FLU test. We do a large volume during the flu season so we run 6 analyzers during the season. The training was simple and easy. The new collection requirements were very easy for our nurses to change. The testing on the analyzer is 2-step which could hold up a single 3rd shift person, but the incubation time is minimal. Test results are better compared to the other lateral flow test. Drawback is that the FLU kits are stored separately, one room temp and one refrigerated. The challenge is to make sure techs are using the correct corresponding kits. You will need lots of counter space for multiple analyzers. "

"The product comes in an easy to use format with little sample preparation. The instrument footprint is small and has a quick turnaround time compared to the current methodology. The after sales care is minimal as all transactions are strictly email or phone service. If analyzer has issues, it is shipped to manufacturer. The cost is comparable to the current method, but will decrease turnaround time significantly and reduce time to treatment considerably. "

Have not had any invalid results yet (which put you in a little bit of a limbo land as to what to do next - retest with another method or recollect, neither very practical options). About the only problem trainees had getting used to was clicking the transfer device hard enough to make the button go up and down."

"The Alere i has on screen step by step instructions, which are very easy to follow. The testing is waived, allowing me to train our nurses to run the test when lab is on call rather than in the building. The training materials have been very helpful and are easily understood by non-lab users. The startup items allow plenty of practice prior to patient testing. Prior to starting patient testing, we performed the Viral panel, the Alere i gave expected results while the Binax Now influenza tests did not even pick up the Influenza. With the change in methodology, we are able to charge a different CPT code and reimbursement is much higher. Support before, during and after purchase is awesome. "

"This product is fairly easy to use. The nature of the testing has allowed our organization to significantly reduce the amount of outside lab cultures we need to perform which is a better benefit for our patients. It is expensive to run a quality control test on each shipment - so I have started to order more test kits per shipment to reduce the number of QC tests needed. The influenza test kits have a component that must be refrigerated, so if space is a problem, you may want to consider that as well. I also really like that the strep tests will generate a positive result earlier than the entire 8-minute run cycle, which is a real time saver when multiple patients need testing. "
