"It is very easy to use, and interpretation of the test result is simple - just observe the blue lines on the Ag and Tox sides of the reaction window. The background is white and gives a discernible result. All staff (3 shifts) have no problem running the tests. In the past, we thought just liquid faecal specimens would be positive for either toxin or antigen. But over the years, liquid, solid, and semi-solid faecal specimens can also give positive toxin or antigen results.The only drawback - we have to bring all the reagents and devices to room temperature before running the tests."

"Since switching from an EIA C.diff test to the Alere Quik Chek C.diff test, the difference has been night and day! We used to batch our tests because they took so long to run, but the Quik Chek allows for rapid testing of individual samples resulting in a much faster turnaround time. We missed several positive C.diff patients using the EIA method due to poor sensitivity for that methodology. I am confident in the results that we turn out using Alere's test kit, plus having the toxin detection built right into the test cassette is a huge time-saver!"

Manufacturer's ResponseMr. Crockett, I'm sorry to hear you are disappointed in the sensitivity of the toxin test. The sensitivity often looks low compared to molecular tests or toxigenic culture. This is because carriers can have the bacteria present without actively producing toxin. They will be positive by PCR or Tox Culture, since the bacteria can make the toxin if needed, but are negative by a Toxin assay because toxin isn't actively being produced. There is nothing in the instructions about how many samples are required to rule out CDIFF. 3 consecutive days of testing is not always convenient or cost effective to keep a patient in the hospital vs other methodologies. However, this test is simple and straightforward if it is the methodology in use."

Due to the increased sensitivity of the GDH antigen, only one sample is necessary to report a patient as negative for CDI. The ASM and ESCMID guidelines support ruling out C. diff with a single negative GDH result with no further testing.

If you are running the 50T kit, you can remove half of the reagents and keep the other half in the fridge. The bottles will also warm up faster if removed from the insulating foam carrier in the box.

"Easy enough to use. We only pick up about 1/2 of our toxigenic C diff from this product. The other half are found after reflex to PCR. Also, wish everything required for testing was in the kit, including tubes."
Manufacturer's ResponseMs. Davis, your results are in line with many publications that show only about half of PCR-positive samples have active toxin production. The remainder are typically carriers who have the bacteria present, but toxin is not being produced to cause disease. All reagents, pipets, and external controls are included in the kit. Tubes are not included due to the variety of tubes commonly used in labs, and the need to have corresponding racks.

"We chose to use an algorithm that started with screening for C diff Ag and toxin. We then will reflex testing to PCR if Ag is detected , but not toxin. We favor this approach over just testing by PCR because it allows us to tell the physician if toxin is being actively produced. It also lets the physician know that a positive PCR test could be attributed to a carrier state instead of C diff disease. We feel that a lot of the current literature is validating this approach. Our algorithm is less expensive than PCR only testing. Another benefit is enhanced antibiotic stewardship."

"I would highly recommend this kit to any other lab. It is very easy to use and our product rep came in and helped us do the validation. It reproduces well. When you can get a result in 25 minutes it is a huge boost for patient care. It gets that patient off of isolation faster, saving the hospital money. We don't have any problems using the kit, but if we did Trey and Robin are only an email away. They are always checking on us to make sure the products are performing as expected. The pricing is reasonable because when you save the hospital dollars on length of patient time on isolation it is worth it."

I have spoken with our Technical Consultants to reach out to you to help resolve any issues. The incubation steps do take time for the sample to interact with the reagents and can't be shortened. "The quick check complete is so easy to use that I was able to take C. difficile testing from testing once/day in a batch to testing as it comes into the lab 24/7. It enables us to make quick decisions about whether a patient needs to be placed on enteric precautions or not. Also, I have correlated this test to molecular results in proficiency testing and there has been 100% agreement each time. Having the antigen and the toxin detection on board 1 cartridge is awesome."

The C. DIFF QUIK CHEK COMPLETE® test (manufactured and developed by TechLab®, Inc.) is the only rapid cassette assay that simultaneously detects both glutamate dehydrogenase (GDH) antigen and toxins A & B of C. difficile in fecal specimens.
The test detects C. difficile antigen, GDH, as a screen for the presence of C. difficile and confirms the presence of toxigenic C. difficile by detecting toxins A and B in fecal specimens from persons suspected of having C. difficile disease. This test is to be used as an aid in the diagnosis of C. difficile disease.
